A loaded logging truck lost braking power while descending a steep grade and utilized a runaway lane that was immediately available. Once the truck was stopped, it did not have enough braking power to hold the truck on the grade of the runaway lane. Workers removing a protective coating from an underground intermediate-pressure gas main were exposed to airborne asbestos fibre and coal tar pitch dusts over a period of about 2 weeks. Traffic lights were being installed by a worker in the elevated bucket of a ladder truck parked in the curb lane. The bucket, raised approximately 12 feet from the roadway, extended approximately 3 to 4 feet into the centre lane and there was no effective traffic control. The trailer of a chip truck travelling in the centre lane hit the bucket and tore it from the ladder. The bucket and worker travelled along the top of the first loaded trailer and came to rest between the two trailers.

A hoist motor hanger failed on one of the two hoist motors supporting a suspended work platform in an elevator shaft approximately 140 feet up. The platform fell away on one side, causing a worker to fall into the shaft. The worker's fall protection equipment arrested his fall. Examination of the failed hanger bracket indicated that it was subjected to reverse bending cycles that initiated cracks at a location where, due to the shape of the bracket, stress concentrations can be expected to occur.

A tri-axle tank truck descended a short hill with a sharp corner at the bottom. As the truck entered the corner, a slippery wet section caused the truck's trailer to slide and pull the unit off the highway. The truck and trailer rolled three times, shearing off a power pole. A pallet jack was being used to move a load of hardwood to the edge of the deck of a transport trailer. A young worker stood on the raised forks of a lift truck, attempting to remove the pallet jack, when the jack's wheel fell off the trailer deck. The upper skid of product (approximately 1 ton) tilted and fell forward, pinning the worker between the load and the mast of the lift truck.

A worker mistakenly picked up a container of concentrated hydrochloric acid and used it to top up a pool treatment chemical vessel containing sodium hypochlorite. When the acid was added to the bleach, there was an immediate release of chlorine gas. The building was evacuated but 14 people were exposed to chlorine gas.

Workers on a drilling rig were experiencing problems with the brake on the draw works (hoisting drum). After repairing a diaphragm in the braking system, the crew attempted to re-set the crown saver device. As they did so, the brake failed and the hoisting blocks dropped to the floor of the drilling rig, narrowly missing two workers.

A log transporter's self-loading boom and grapple were positioned on top of the load of the trailer to provide greater traction. When the truck drove under a 25-kV power line, it contacted a lower telephone line (19 feet above ground), which then caused an adjacent utility pole to break off. The pole and energized 25-kV line landed on the loaded truck.

Mercury (1 teaspoon) was discovered on top of a fire-hose cabinet. The area was isolated and the spill was cleaned up by a qualified worker. Subsequent monitoring did not detect mercury vapour. The mercury may have come from a mercury switch that had been removed for maintenance and possibly placed on the cabinet.

As a house was being moved along a highway, the roof ridge contacted and snagged an overhead telecommunications cable bundle. The cable bundle was attached to a power pole that also supported an energized 25-kV power line running parallel to the highway. The pole to which the cable bundle was attached and two other poles (one on either side) broke off. The middle pole and still-attached electrical conductors came to rest atop the house.

Workers were removing clinker (sand and silica deposited during the combustion of hog fuel) from one wall of a power boiler. The workers were inside the boiler furnace when a 2000-pound chunk of clinker came loose from the wall and fell. The falling chunk crushed the scaffold set up for worker protection during access and egress through the manway. One worker jumped clear of the falling debris; two other workers were about five feet away when the incident occurred.
