A worker had been fabricating an I-beam using a jacking frame and a portable powered hydraulic ram to bring the two flanges tight to the web. A 5-inch-long piece of square tubular steel was used as a spacer between the base of the ram cylinder and the frame. Initial evidence, including a split hard hat, indicates that the jacking frame and ram configuration failed under pressure, causing a blow to the worker's head.

A plumber fell 10 feet (to ground level) in an elevator shaft in a house under construction. The entrance door to the elevator shaft was effectively guarded, but the plumber stepped between the wall studs (16-inch centres). The worker may have thought he was stepping through the stud wall into the adjacent room, which was poorly lighted.

As a mini-yarder attempted to lift a turn, the anchor stump on one of four guy lines pulled out of the ground. This caused a second guy line to pull off the top of its anchor stump. The yarder was pulled over and the yarder's fairlead hit a loader, breaking off its guarding. The side and front windows of both machines were broken.

Two logging skidders were pulling and pushing a loaded super B-train log truck, trying to bring the truck out of the loading area, up a hill, and onto the main logging haul road. As they proceeded down the haul road, the truck's trailers slid to the left, and the roadway shoulder collapsed under the pup's driver-side wheels. The whole unit slid into a ditch about 4 feet deep, then the trailers and truck turned over onto their side, injuring the driver.

In preparation for cutting a 30-inch pipe, workers tore away a piece of insulation that was covering it. The insulation contained asbestos, and both the elevated work platform and surrounding work area were contaminated. Workers did not breathe in any asbestos fibres because they were using supplied air to protect them from other hazards.

Two electrical workers were trying to determine the source of a major power outage. To access a remote road, they had to go through a gate. One worker moved their vehicle forward through the gate while the second worker went to close the gate. The first worker (vehicle driver) put the vehicle in "park," then slid over to the passenger side. The vehicle rolled back, striking the heavy steel gate and driving it into the chest of the second worker.

As a grapple skidder was getting into position to pick up a drag of logs on a 15 percent sloped hillside, the machine was high-centred on several large boulders. When the operator tried to clear them, the skidder slid off the boulders and rolled over several times.

A tower crane deposited a load of bundles of mechanical pipe and ready rod onto a dolly on a landing platform on the 34th floor. The choker slings were unhooked and pulled under the load. The two eyes of one choker sling were attached to the load line hook, but only one eye of the other sling was attached; the second eye was left hanging. The tower crane operator was radioed to raise the load line in a blind lift. As the crane lifted its load line, the eye that had been left hanging caught on one end of the load. Most of the bundles fell to a catch platform on level 25, and some pieces bounced off the platform and landed in the storage yard at grade.

A 165-ton lattice-boom truck-mounted crane was lifting a tilt-up panel wall weighing 63,000 pounds. With the panel almost vertical, the boom was lowered to place the panel on the footings. The two outriggers farthest from the panel started to lift off the ground. As the panel fell outwards, the operator lowered the load line to prevent the crane from tipping over. By the time the panel was horizontal on the ground, the crane body had been lifted to approximately 45 degrees.

Workers were setting up and preparing a sour gas well to conduct a slickline well-servicing operation. The lubricator for the slickline had been positioned and the wellhead had been purged with nitrogen. To facilitate the entry of the deadweight into the wellhead, a worker climbed up onto the wellhead. As the main wellhead valve was opened, a faint hissing was heard. The worker on the wellhead (a young worker) collapsed as a result of exposure to hydrogen sulfide (H2S) gas.

A worker was taking pump readings while a dump truck with tandem trailer waited before moving ahead (8-10 feet) to the pumps. The worker finished taking the readings, waved to the driver, then started to cross in front of the truck. The driver checked the truck's mirrors to ensure that the trailer would clear the pumps. He then looked ahead and since he didn't see the worker, proceeded ahead to fuel up. Meanwhile, the worker had spotted some glass on the ground and knelt down to pick it up. As the truck moved forward, it knocked the worker over and rolled onto one leg.
