A worker was assisting an emergency-room physician to insert an intravenous needle in a patient who was unstable and had already removed numerous previous intravenous devices. The patient was also actively seizuring at the time of the intravenous initiation and the primary focus was on securing the intravenous site to ensure that the necessary medications could be administered. The physician withdrew the needle before activating the safety device. As the worker activated the saline lock, one of the worker's fingers sustained a needlestick injury.

When a lift truck became stuck on a soft surface, the operator connected the mast to the side rails of a transport truck trailer with a wire rope sling. The transport was positioned perpendicular to the lift truck. The transport driver pulled forward to tow the lift truck. As the lift truck started to move, the operator lifted the forks to prevent them from striking the side of the transport. The lift truck, now being pulled from above its centre of gravity, flopped over onto its side. The operator was trapped under the canopy structure.

During placement of concrete infill for a masonry block wall, the 2-inch-diameter rubber hose of the concrete line pump became plugged. After the pump was shut off, workers tried to unplug the blockage. Back pressure in the line released unexpectedly, causing the unrestrained end of the rubber hose to whip. The hose struck the head of a young worker, but the worker was wearing safety headgear and was not seriously injured.

The hopper had been removed and the diverter system (Y gate vane feeder), supported on a box beam, was being disconnected from the ducting. When the bolts that connected the ducting to the diverter were cut, the diverter tipped on the support beam and fell onto the worker. The worker was pinned between the 470-kg (1,034-pound) diverter and the platform guard rail. The injured worker was removed from the platform by the high-angle rescue crew of the local fire department.
