About an hour later the utility provider authorized another valve location to be closed.

The contact resulted in the utility pole breaking and falling to the roadway, causing acid to leak from the transformer onto the road. A worker was operating a feller buncher on 20 to 30 percent slopes. The tracks of the feller buncher slipped on a rock outcropping, causing the machine to roll over and land upside down. The operator's cab escape hatch and door were wedged shut by earth and stumps. A fire started in the engine compartment, subsequently engulfing the cab. Attempts to suppress the fire were unsuccessful.

A tow truck was removing a scrap car from the back yard of a private residence. The car was being dragged so the tow truck operator worker went under the vehicle to disengage its transmission. Once the transmission was unhooked, the vehicle suddenly rolled forward, crushing the worker under the front of the vehicle. A neighbour who witnessed the incident was able to lift the vehicle off the worker using the tow truck's winch.

Workers were cleaning under slip forms designed and used to cast concrete fence panels. A forklift with a makeshift widener added to the forks was being used to lift the slip form panel from the narrow end. The operator (a young worker) exited the forklift, leaving the load suspended 6 to 8 inches. The panel slipped from the forks and struck the operator, pushing him into the adjacent panel.

Two workers were aligning the tow bar of a compressor when one of the compressor's wheels dropped off a shallow lip. The compressor swung toward one of the workers, pushing him against the side of a slow-moving concrete delivery truck. The worker was knocked to the ground and the wheels of the truck moved along his leg to the waist. The other worker stopped the truck and directed the driver to back off of the worker.

A non-worker in a motorized wheelchair was being guided by an aid worker. The wheelchair struck a heavy, temporary sidewalk covering plate above a 3-foot-deep excavation, pushing the cover plate forward. The wheelchair and occupant, as well as the aid worker, fell into the excavation. The sloped cover plate had been resting on the top edges of a cut concrete sidewalk, but the remainder of the excavation (in front of the sidewalk) was open. The area was marked with warning tape and municipal signage/flashing lights.

An excavator was conducting cleanup work at the site of a residential fire. The excavator operator placed a spare excavation bucket on the ground adjacent to the work area. When the cleanup work was completed, the worker tried to retrieve the spare bucket and inadvertently pulled the cap off a half-inch gas riser sticking out of the ground. The worker was unaware that the gas riser was there as it was not marked after the utility company had removed the gas service meter to the home that used to be at that location.

During a bulk excavation, a worker from the power utility company noticed that a utility pole looked unstable. The worker reported this and a crew came out and stabilized the pole. The company doing the excavation hired an engineer who gave instructions on how to further stabilize the pole with dirt. While carrying out his instructions, someone removed the anchor that the utility company's crew had placed. The crew was called back to re-stabilize the pole. The crew also painted a "no work zone" around the pole, pending its removal.

During the clearing of an 8-inch sewer line, a camera was sent through the line to observe the problem. The camera could not get past a root system some 140 feet from the manhole. The root-sawing system was sent into the line and began to remove the roots. The camera could not see a gas line behind the root system, and during the root-sawing operation, the 19-mm gas line was severed. Work stopped and the area was evacuated. When the area around the gas/sewer line was exposed, it was discovered that the gas line had been installed through the sewer line.

The conveyor motor tripped out but the worker was unable to free himself. He was pinned in this position for about 15 minutes, until he was able to use his cell phone to contact a family member who then contacted the mill.

A tractor-trailer failed to stop at a stop sign and entered the highway making a left turn in front of a public transit bus. The tractor-trailer contacted the right rear tires of the bus, causing the bus to fishtail. The transit operator was able to keep the bus from going into oncoming traffic and it then ended up on an embankment south of the intersection.
