A new storm drain main was being installed in a municipality. Service gas lines were fed from a gas main running parallel to the storm drain main. The gas main and 13 of 14 gas service lines were marked on drawings before excavating began. A corner lot that was serviced from the service corridor on one road but with a civic address on the other road was missed during the locate. During excavating, a 3/4-inch service line was severed. The utility company was contacted and repairs were made. There were no injuries.

A traffic control person (TCP) was working at an intersection late at night. The TCP was struck by the left side-view mirror of a car driven by a drunk driver. The worker sustained extensive bruising and lacerations. A second TCP narrowly escaped being hit by the drunk driver's car. The driver remained at the scene and was taken into police custody.

While excavating at a construction site, an excavator operator uncovered a stick of unfired explosive. The finding was reported to the site supervisor, who eventually removed the stick of explosive. The site supervisor was not a certified blaster. A certified blaster should have been called to examine the site and determine if there were any other signs of explosives on the work site, as well as to properly dispose of the found explosives.

Three workers were changing blades on a vertical bandmill resaw. One of the workers was lowering the saw cradle through an opening in the floor above the resaw. He fell through the opening and landed on the saw frame about 14.5 feet below, then fell another 9 feet to the conveyor chuting below the frame, and finally fell another 3.5 feet to a running box-link waste conveyor. He travelled about 20 feet on the conveyor before it was stopped by another worker.

Workers on a drilling rig were removing the drill string from the well to replace the mud motor (hydraulic downhole device that turns the drill bit while drilling horizontally). A snubbing unit (attachment used to move drill pipe or tools into or out of a well while under pressure) was being used. The drill bit was being drawn into the BOP stack (series of blowout prevention valves attached to the well, used to control well pressure while drilling) when a sudden volume of sweet natural gas (natural gas not containing hydrogen sulfide) escaped from the annulus. The gas cloud ignited within seconds. Workers initiated emergency procedures to shut in the well, but the drilling rig was destroyed in the fire. One worker was injured.

A mercury sphygmomanometer (blood pressure measuring unit) was donated to second-hand store for resale. When a customer picked up the unit, it separated, resulting in a loss of mercury. Customers were evacuated, then the four workers went to a room where their footwear was retained. The area of the spill was barricaded and spill-handling procedures for mercury provided on WorkSafeBC's website were consulted. The Ministry of Environment and a spill response team were contacted, then clean-up was initiated by the spill response team. About 20 grams of mercury was recovered.

Two workers at an oil and gas compressor site were running a meter proving test (a weights and measures requirement for sales of oil and gas products) when one of the high-pressure metering hoses ruptured and broke in half. The high-pressure hose then flailed around, striking one worker and spraying sour condensate (a light hydrocarbon liquid entrained with H2S gas) onto both workers.

A utility company hired a subcontractor to remove beetle-killed trees adjacent to energized high-voltage power lines. A young worker climbed a 14-inch diameter tree (about 65 feet tall) to cut limbs between that tree and a smaller tree growing close to it, because the two trees were limb tied. After removing the connecting limbs and the top of the smaller tree, the worker started to descend the tree. At about 39 feet above the ground, the worker felt movement in the tree as the tree began to uproot. The tree then fell over downslope into an adjacent energized high-voltage (25-kV) power line.

The fall restraint rope grab did not deploy as it was now installed upside down.

A volunteer was moving a rubber-tired crane down a ramp (grade of 25 percent) into an underground parking area under construction. An excavator was assisting to move the crane. The excavator was placed at a severe angle to the ramp due to a pile of building materials in the area. When a snubbing strap became tight, the crane's rear wheels were pulled to the left off the ramp. The crane rolled onto the cab side, trapping the operator inside and impaling him on reinforcing steel dowels. The operator was extracted from the cab by emergency personnel.

A tower crane was offloading sections of a man-lift mast from a delivery truck at a designated worksite load/offload area. The area was in the curb lane of a street with overhead, rubber-guarded, and flagged trolley bus electrical lines (600 volts DC). The tower crane offloaded one mast section from the delivery truck deck and placed it on the ground. The crane operator then tried to trolley and slew the tower crane load block back to the delivery truck to lift a second mast section. During this movement, the 3/8-inch by 20-foot lifting chains attached to the tower crane load block contacted the rear (back) area of a transit bus travelling in the second (middle lane). The crane operator quickly attempted to slew and trolley the tower crane, causing the lifting chains to contact the rubber-guarded trolley bus electrical lines located over the curb. No one was injured and no electrical arc occurred.

On a weekend, two maintenance workers replaced the solenoid valves on two evaporators in a "chiller room" (which houses ammonia refrigeration equipment) of a poultry processing plant. As the workers left the room, a highly corroded carbon steel bushing (threaded reducer) ruptured in the hot ammonia gas line strainer assembly for one of the other evaporators, causing a major ammonia gas leak. The two workers, and five other maintenance workers who were on shift, evacuated the building and called emergency services. The fire department, police, and ambulance service responded. An emergency hazardous materials response provider assisted the refrigeration contractor to isolate the leaking ammonia refrigeration system, change out the corroded bushing, and purge the processing plant of ammonia gas. The ammonia leak was not fully isolated until more than 12 hours after it began. No workers required treatment for exposure to ammonia but there was a risk of serious injury or death.

The blotter rolled forward and dropped 6 feet to the floor, grazing the second worker and striking the third worker.

While a mechanical head was processing logs, a cutting chain link broke away from the processing head bar at high velocity and cut through the equipment's 12-millimetre polycarbonate front windshield guard. The chain link hit the back of the steel internal computer box, denting it, then dropped to the floor. The processing head had a manufacturer's chain catcher installed. The polycarbonate guard was rated to the G603 standard.

A crew cab was travelling on a forest service road when the driver attempted to pass a slow logging truck. As the crew cab pulled out, the steering box pitman shaft fractured completely. Out of control, the crew cab ran into the back of the loaded logging truck. The three workers inside were able to duck under the dash of the crew cab before the log ends broke through the windshield.

Two workers had erected metal tubular scaffolding to a height of 10 feet and set up a 22-foot extension ladder on the top of the scaffolding. One worker stood on top of the scaffold, holding the base of the ladder, while the other worker climbed the ladder. The scaffolding became unstable and abruptly leaned to about a 45-degree angle before coming to rest on a small tree. Both workers fell to the ground.
