The company doing the excavation hired an engineer who gave instructions on how to further stabilize the pole with dirt. While carrying out his instructions, someone removed the anchor that the utility company's crew had placed. The crew was called back to re-stabilize the pole. The crew also painted a "no work zone" around the pole, pending its removal.

A live line puller/hoist was being used to pull the conductor into the splicing sleeve. Workers were dismantling a dust collection system from a platform about 35 feet above ground. The hopper had been removed and the diverter system (Y gate vane feeder), supported on a box beam, was being disconnected from the ducting. When the bolts that connected the ducting to the diverter were cut, the diverter tipped on the support beam and fell onto the worker. The worker was pinned between the 470-kg (1,034-pound) diverter and the platform guard rail. The injured worker was removed from the platform by the high-angle rescue crew of the local fire department.
