Workers at a completed gas well (hot oiling crew) were heating water to prepare to pump a water frac downhole (a process used to fracture underground formations to enhance gas flow). Just prior to pumping the frac water, a flash fire occurred in one of the 400-barrel storage tanks on-site. The force of the ignition deformed the tank top and blew steam and smoke out one riser. No workers were injured.

A worker was driving a pickup truck towing an air trailer (a trailer used to supply air to breathing apparatus at oil and gas well sites) on a highway. As the driver attempted to pass a transport truck, he lost control of the pickup and rolled into the ditch.

While loading and testing a perforating gun (a device fitted with shaped charges or bullets which is lowered into a well and fired to create penetrating holes in the well bore), a worker mistakenly attached a blasting cap to the switch being tested. The exposed cap detonated and the worker sustained minor cuts from the metal cap fragments.

A drilling rig crew was drilling an oil and gas well when a kick (influx of formation fluids under pressure into the well bore) occurred at about 1850 metres total depth. Well-control procedures were initiated and the kick was circulated out without injury.

On an oil and gas site, during a coil tubing operation (using a continuous steel pipe for well completions) in which the unit was pulling the pipe out of a gas well, the drive motor for the coil tubing unit stopped running. The loss of power then resulted in the loss of hydraulic power/pressure and a subsequent loss of control of the pipe. The pipe fell back down the well, resulting in possible shock loading to the attached crane.

After completing a connection on the drill rig floor, a worker operating the breakout side tongs (winch-operated wrench located on the far side of the rig floor, used to tighten/loosen threaded pipe connections) tried to secure the tongs in the idle position. The worker requested additional slack in the line and when activated the tongs swung back and struck the worker in the chest.

An avalanche estimated to cover 1.5 hectares and to be 10 metres deep came down on a forest service road. The slide mostly buried the pickup truck of a worker who had just arrived at the site. The worker was able to exit the truck through the driver's side window.

A feller/buncher was on a slope in a cut block, harvesting trees. The feller/buncher started to slide down the slope and struck a tree stump, causing it to flip on its side. The feller/buncher continued to slide, rolled onto its roof, and caught on fire. The operator exited the cab in the early stages of losing control and escaped without injury.

A worker on a snubbing unit (machine used to hydraulically move pipe in or out of an oil or gas well) was using a controlled-descent device to get down to the ground when the clip attached to the descent device detached. The worker then fell about 5 metres to the ground.

A drilling rig crew was preparing a drilling rig for a move to a new location. The crew had secured the travelling blocks (hoisting blocks used to hoist the drill string) and the top drive unit (device used to turn the drill pipe) by chaining the blocks to the rig's derrick. After the travelling blocks had been secured, the crew removed the wire rope drill line from the draw works (hoisting drums of the drilling rig) and coiled the line on the rig floor. As the line was being coiled on the rig floor, the chain securing the blocks broke, allowing the blocks and top drive to fall. The falling blocks rapidly drew the coiled line back up into the derrick. A worker on the drill floor became entangled in the line, resulting in a serious injury to his leg.

A worker was conducting maintenance on a boiler used to provide steam and heat to activities on an oil and gas wellsite. The worker was blowing down the boiler and when the auto restart process was initiated, an explosion occurred in the fire tube. The worker was beside the boiler and was knocked down by the concussion.

A drilling rig crew was running drill collars (large pipes used in the drilling process) into the drill hole. The crew was using a rope to swing the pipes into position to allow them to stab the elevated joint of pipe onto the joint of pipe positioned in the well bore. A young worker was standing to the side of the pipe when the rope slipped out of his hand. In an attempt to stop the elevated joint of pipe from moving farther, the worker tried to stabilize the pipe with his hand. His hand was pinched between the drill collars and the mechanical pipe connector (iron roughneck).

A feller buncher tipped over onto its side while exiting the logging block at shift end. The machine was working (side sloping) on a 15-degree slope and encountered wind-felled trees. The operator's hand was crushed between the window and a handhold, due to the machine settling on a stump. The operator, a young worker, used the roof escape hatch to exit the machine.

A crab fishing vessel had just retrieved the last trap on board and made a porthand turn. During the turn, a series of large waves spilled onto the deck, laying the vessel over. Gear on the deck slid and blocked the scuppers, preventing the water from exiting the deck. Water then began to flood through the open galley door. The three crew members were able to depart in a life vessel before the crab fishing vessel sunk.

While a mechanical head was processing logs, a cutting chain link broke away from the processing head bar at high velocity and cut through the equipment's 12-millimetre polycarbonate front windshield guard. The chain link hit the back of the steel internal computer box, denting it, then dropped to the floor. The processing head had a manufacturer's chain catcher installed. The polycarbonate guard was rated to the G603 standard. A crew cab was travelling on a forest service road when the driver attempted to pass a slow logging truck. As the crew cab pulled out, the steering box pitman shaft fractured completely. Out of control, the crew cab ran into the back of the loaded logging truck. The three workers inside were able to duck under the dash of the crew cab before the log ends broke through the windshield.

Two workers at an oil and gas compressor site were running a meter proving test (a weights and measures requirement for sales of oil and gas products) when one of the high-pressure metering hoses ruptured and broke in half. The high-pressure hose then flailed around, striking one worker and spraying sour condensate (a light hydrocarbon liquid entrained with H2S gas) onto both workers.

The equipment was lifted using a tractor with a 3-point hitch and propped up with a piece of wood (6.25 inches wide, 2 inches thick, 24.75 inches long). While the worker was underneath the mower, the wood kicked out and the equipment dropped, crushing the worker. The equipment weighed 1,200 to 1,500 pounds.

A worker was operating a feller buncher on 20 to 30 percent slopes. The tracks of the feller buncher slipped on a rock outcropping, causing the machine to roll over and land upside down. The operator's cab escape hatch and door were wedged shut by earth and stumps. A fire started in the engine compartment, subsequently engulfing the cab. Attempts to suppress the fire were unsuccessful.

A forklift was moving loads of manufactured wood when the two bottom anchor points on a forklift's mast failed. When the operator reversed the forklift, momentum caused the mast to strike the cab. The mast punctured the front window and the hydraulic cylinder dented the top of the cab.
