A municipal worker drove a recycling truck to the city refuelling station. The truck hit a pillar while travelling at less than 10 km/h. The worker fell from a 1-m (3-foot) step inside the truck cab and struck his head on the concrete ground.

During backfilling around stand pipes for a municipal water supply upgrade, a worker was struck from behind by a large piece of clay (about 90 kilograms). The worker had been wrapping a newly installed valve in the same excavation.

A new storm drain main was being installed in a municipality. Service gas lines were fed from a gas main running parallel to the storm drain main. The gas main and 13 of 14 gas service lines were marked on drawings before excavating began. A corner lot that was serviced from the service corridor on one road but with a civic address on the other road was missed during the locate. During excavating, a 3/4-inch service line was severed. The utility company was contacted and repairs were made. There were no injuries.

Workers were removing and chipping a tree. The top portion of the tree had been removed. The remaining standing piece of the tree, 8 to 10 inches in diameter and 19 to 21 feet tall, was being removed by a worker using a chainsaw. A second worker was feeding the removed top into a chipper. As he walked to the end that had been positioned farthest from the chipper, to align the top with the chipper, he was struck by the remaining piece being removed.

A worker was ripping (cutting lengthwise) a 12-foot pressure-treated 2x4 on a 10-inch table saw. As the operator reached around the saw blade's safety guard to hold the 2x4, one finger contacted the saw's blade.

A new dehumidifier unit had recently been installed in an indoor swimming pool complex. The dehumidifier uses highly pressurized refrigerant. A rupture in an extruded copper pipe "T" fitting caused refrigerant to be released into the pool area while the pool was open for public swimming. The pool complex was evacuated and the fire department and paramedics were called. Of the five pool workers on duty, two (both young workers) reported symptoms of exposure to the refrigerant.

After hose practice, a fire crew lifted the fire truck ladder in order to stow away the hose. The ladder was not lowered again, and it contacted overhead lines when the truck exited the fire hall parking lot.

A non-worker in a motorized wheelchair was being guided by an aid worker. The wheelchair struck a heavy, temporary sidewalk covering plate above a 3-foot-deep excavation, pushing the cover plate forward. The wheelchair and occupant, as well as the aid worker, fell into the excavation. The sloped cover plate had been resting on the top edges of a cut concrete sidewalk, but the remainder of the excavation (in front of the sidewalk) was open. The area was marked with warning tape and municipal signage/flashing lights.

During garbage collection, a worker noticed a broken weld on the left cart lift mechanism. As the worker manually lifted the mechanism to inspect the damage, the mechanism was lifted past the stop and was caught by the closing packing blade of the garbage truck. This caused the lift mechanism to break away from the attaching bolts (holding the upper part of the mechanism) and strike the worker.

During the clearing of an 8-inch sewer line, a camera was sent through the line to observe the problem. The camera could not get past a root system some 140 feet from the manhole. The root-sawing system was sent into the line and began to remove the roots. The camera could not see a gas line behind the root system, and during the root-sawing operation, the 19-mm gas line was severed. Work stopped and the area was evacuated. When the area around the gas/sewer line was exposed, it was discovered that the gas line had been installed through the sewer line.

A backhoe operator was struck on the head by an auxiliary tooth while operating from the rear seat. The tooth dislodged from the bucket while the "thumb" was restraining a length of 2-inch copper pipe that was being pulled from the ground.

A worker was pruning trees along a roadway using a tripod orchard ladder set up on concrete. A large branch that was being pruned fell onto the ladder, causing it to collapse and the worker to fall to the roadway.
