The address given is the location where the violation occurred and is not necessarily the address of the penalized business. The registered business name is given as well as any "doing business as" (DBA) name.
The delay between the date of the penalty and publication of the penalty by WorkSafeBC is partially a result of allowing time for employers and other interested parties to respond to the initial penalty and for the appeal process.

Fatality. During road construction, this firm failed to adequately instruct, direct, and supervise workers and to ensure that safe work practices were followed as required during blasting operations. In particular, the firm permitted a blaster to proceed with unsafe work practices in preparing and attempting to fire a blast. The firm failed to prepare, maintain, and ensure adequate written blasting procedures. Attempts to fire a blast were made using, first, a flashlight battery and then a truck battery. Further, a previously discarded and damaged firing line was used to connect the power source with the explosives. Continuity in the firing line was tested using a 12-volt light bulb taken from a rock truck. During these tests on the firing line, the blast detonated. A blaster within the blast area was fatally injured. Other workers were in the area impacted by fly rock but escaped unharmed.
