Penalties - Construction
Penalties are fines for health and safety violations of the Workers Compensation Act and/or the Occupational Health and Safety Regulation. For information on when penalties are considered and how the amount of the penalty is calculated, see the FAQs.
The address given is the location where the violation occurred and is not necessarily the address of the penalized business. The registered business name is given as well as any "doing business as" (DBA) name.
The delay between the date of the penalty and publication of the penalty by WorkSafeBC is partially a result of allowing time for employers and other interested parties to respond to the initial penalty and for the appeal process.

The walls of the excavation were nearly vertical, and a compactor machine was causing vibrations, which could increase the likelihood of a collapse. There were large piles of dirt beside the trench, which could have fallen into the trench in the event of a collapse.

Workers performed work inside an unsloped, unshored excavation, which was more than 2.4 m (8 ft.) deep, 1.2 m (4 ft.) wide, and about 30.5 m (100 ft.) long. The soil in and around the trench was soft, sandy material that was known to be prone to sloughing. The trench-area soil presented a high risk of injury or death to workers inside the excavation. The firm had been cited for a similar, prior violation and had previous knowledge of the regulatory requirements. The firm directed its workers to enter into the excavation, knowing that the worksite was in violation of regulatory requirements. Firm representatives stated that the reason for taking such actions was because the work's anticipated duration was "short term" and in this case expected to take two days. The firm has a history of high-risk violations, committed willfully or with reckless disregard.

adequate supervision, instruction, and information to perform their duties safely.
Workers were not provided with demolition and truss salvage safe work procedures.
The employer did not conduct a hazardous materials survey and make the results
available to workers. The safe housekeeping of waste materials was not maintained.
The owner/employer was not providing a workplace in accordance with the Occupational
Health and Safety Regulation and the Workers Compensation Act.

A tree had also been felled onto an energized high-voltage power line.
