2003 Incidents Involving Young Workers

A Notice of Accident (NOA) is a preliminary report of an accident that WCB Prevention officers respond to. The NOA is based on information obtained as soon as possible after the accident but before an investigation into causes has been completed.

Two workers experienced disconnection of their supplied air respiratory protection system while they were removing sludge from the inside of an 80,000 barrel (12 719 cubic metre) crude oil tank. They attemped to exit the confined space, without either worker activating his emergency egress bottle. One worker was exposed to the vessel atmosphere when he removed his face-piece inside the tank; the young worker banged his head on the entryway.

Two young workers were removing debris from a 7-foot-deep sump (part of a drainage system used to collect rainwater from a parking lot). During the sump cleaning, one worker would enter the sump to fill a bucket with a shovel, and the second worker would stay outside the sump to assist with removal of the bucket. During the second day of removal both workers reported to the first aid attendant complaining of weakness, faintness, vertigo, nausea, and a burning throat.

He fell through the stairwell opening and landed on the basement dirt floor approximately 9 feet below.
