A worker was seriously injured when he fell from the bucket of an elevated lift while removing banners. The lift became unbalanced, causing it to tip over. Outriggers used to balance the lift were available but not used. The manufacturer's instructions on the lift indicated that the outriggers must be attached when using the lift.
