A worker was using a cutting torch to cut off a length of salvaged natural gas pipe. He was
cutting from the downhill side of a pipe that was under tension (apparently he didn't realize
that it was under tension). He was almost through the cut when the pipe broke and sprang
back on him, striking him on the chest. He later died from his injuries.
Prevention alert:
- Develop a system to identify pipe under tension.
- Properly restrain any pipe that is under tension.
- Cut from the safe side of the pipe when it is under tension.
