Truck driver crushed by logging truck

A truck driver was killed when the off-highway logging truck he had been driving rolled over him.

The driver had parked the truck facing uphill on a 5% grade, set the trailer brake hand valve, and then climbed down to prepare the truck for unloading. When the truck rolled backwards, he sustained massive head injuries.

The accident investigation found that the vehicle's "maxi" (parking) brakes had not been set.

Safe work practices:
  • Always set parking brakes prior to exiting a vehicle.
  • Ensure that drivers receive adequate training and supervision.
  • Provide drivers with written safe work procedures for truck operation.
