Two workers were on the roof of a double-wide mobile home.
The two halves were a short distance apart and the workers
were placing a tarp across the opening as it was starting
to snow. Instead of proper fall protection, one worker attached
a rope to his tool belt and the other tied a rope around
his waist. The other ends of the rope were attached to the
roof.
When the workers were ready to get down from the roof,
they found that the ropes were too short to reach across
the ladder. They untied the ropes from their waists and
slid down the snowy 5/12 pitch roof. One worker was not
able to stop and slid over the edge, falling more than 3
metres (10 feet). He was seriously injured when he struck
the frozen ground.
- Provide workers with a fall protection system if they
could fall 3 metres (10 feet) or more. For a sloped mobile
home roof, the most practical option is a fall protection
system, which typically includes a full-body harness and
a lifeline attached to an adequate anchor.

- Locate anchors to minimize the fall hazard when workers
connect to and disconnect from the anchor. For double-wide
mobile homes, consider installing an anchor at the split
that can be accessed by ladder.
- Plan a safe way for workers to get on and off the roof.
- Instruct and train workers in the use of the fall protection
system.
- Ensure that workers use the fall protection system provided.
