     <title>Top blew off 400-barrel tank when filling process caused pressure</title>

       Workers were filling a 400-barrel tank with hydrocarbon fluids (condensates) when the filling process caused a buildup of pressure in the tank. This overpressuring of the tank resulted in the top of the tank blowing off. No workers were injured.

       During gas well servicing operations, a coil tubing unit was pulling the coil tubing (a continuous string of steel pipe) out of a pressurized well. A kinked section of pipe was exposed and gas was observed leaking from the kinked section. The blowout preventer was closed and shortly after, the shear rams were closed, which stopped the leaking gas. No workers were injured.

The opening of the vent line resulted in the worker being directly exposed to a release of sour vapours (H2S gas) venting from the tank. The worker experienced nausea and dizziness and was transported to hospital for treatment.

       While working on a construction project in an old rock quarry, a blasting crew exposed some undetonated explosives believed to be from the time the quarry was developed. Primed explosives were destroyed by the blaster of record. The next day the blasting crew discovered a loose detonator and two sticks of explosives. The site was shut down, and WorkSafeBC attended to assist the blaster of record with safe work procedures to clear the site of unfired explosives before work resumed.

The three crew members were able to depart in a life vessel before the crab fishing vessel sunk.

       A young worker lifted a hinged boot guard on the tail spool of a belt conveyor. The worker then used a stick to sweep sawdust out from underneath the tail spool. The worker's sleeve was caught by the belt and his hand and the stick were pinched between the belt and the floor.
